Rates
Rates for psychotherapy and other services may vary. Please call for rates for the specific services you are interested in.
Sliding scale rates may be available.

Insurance
I accept Blue Shield, Aetna, TriWest, Magellan, Magellan EAP and Lyra.

Services may be covered in full or in part by your health insurance or your employee assistance plan. Please check your coverage carefully by asking the following questions:

• Do I have mental health insurance benefits?
• Does my insurance plan subcontract out for behavioral health services, and if so with which company?
• What is my deductible and has it been met?
• How many sessions per year does my health insurance cover?
• What is the coverage amount per therapy session?
• Is approval required from my primary care physician?

Payment
Cash, checks and credit cards ($150 or above) are accepted for all services at the time of service. A retainer is required for child custody related services.

Cancellation Policy
If you do not show up for your scheduled appointment, and you have not notified me at least 24 hours in advance, you will be required to pay the full cost of the session. Insurance companies do not cover missed sessions.
